I went there for orthodontic treatment, but they suggested BOTOX instead. I have consulted more than five clinics, and this was the only one that recommended Botox. In my opinion, this shows a lack of basic professional ethics!

Dear Ms. Müller,
Thank you for taking the time to share your feedback. We take every review seriously and would like to offer some important clinical context.
During your consultation, our assessment indicated signs consistent with craniomandibular dysfunction (CMD), specifically masseter muscle hypertrophy and elevated masticatory muscle activity. In this clinical picture, the masseter muscle exerts excessive compressive force on the temporomandibular joint (TMJ), which can cause TMJ pain, headaches, and in some cases tinnitus.
According to current evidence, botulinum toxin (Botox) injected into the masseter is the most effective intervention for reducing pathological muscle activity in this context. Multiple peer-reviewed studies support its use for TMJ-related myofascial pain (Guarda-Nardini et al., J Oral Rehabil 2012; Sidebottom & Patel, Br J Oral Maxillofac Surg 2013; Ernberg et al., J Oral Facial Pain Headache 2011). It directly addresses the source of the problem, the overactive muscle.
We want to be transparent, as orthodontists, we do not administer Botox and therefore receive no financial benefit from this recommendation. We refer patients to qualified specialists for this treatment. Our incentive is solely your clinical outcome.
We also want to address a common misconception, a standard occlusal splint (Knirscherschiene), while covered by statutory insurance, is designed to protect the teeth from grinding, it does not reduce muscle activity. In cases of masseter hypertrophy, it can in fact intensify muscle loading on the TMJ and worsen symptoms. This is well-documented in the literature (Dao & Lavigne, J Orofac Pain 1998; Türp et al., Clin Oral Investig 2004).
We understand that a recommendation that differs from other clinics can feel confusing or unsettling. However, our obligation is to recommend what the evidence supports, not what is most commonly offered or what generates revenue for our practice.
We genuinely wish you the best and hope you find the right treatment for your symptoms. We remain happy to answer any further questions.
With kind regards,
Dr. Ph. F. Gebhardt
